Job Description

The Director of Case Management is responsible for the implementation of the Case Management and Resource Management Program. Oversees the effectiveness of the RN Case Managers and social workers; reviews the appropriateness of discharge planning. Reviews the effectiveness of the case management program as it relates to length of stay, readmissions, and other quality of care measures.

RINCIPAL DUTIES AND JOB RESPONSIBILITIES:

  • Correctly identifies avoidable days and maintains a system to communicate effectively to UM committee.
  • Competently coordinates the overall discharge planning and social services function, ensuring compliance with federal and state laws and Joint Commission on Accreditation of Hospital.
  • Enthusiastically accepts client referrals and responds accordingly.
  • Appropriately maintains a liaison with community agencies offering services to discharge patients.
  • Successfully ensures patients receive recommended follow-up services.
  • Accurately identifies appropriate cases rendering referrals to Child Protective Services that fulfill the legal requirements for reporting.
  • Correctly interprets rules and regulations set by JCAHO, Virginia Health Department and other outside agencies.
  • Maintains Case Manager schedule and effectively deploys team members throughout the facility.
  • Is responsive to requests for reports, data, and recommendations on Case Management operations.
  • Completes other duties as assigned and proactively anticipates the needs of other team members.

KNOWLEDGE AND SKILLS REQUIRED:

  • Ability to demonstrate excellent communication skills and ability to work collaboratively with all members of the health care team

EDUCATION AND EXPERIENCE REQUIRED:

  • Associate degree in Nursing from an accredited School
  • Three years nursing experience
  • UR or Case Management experience

EDUCATION AND EXPERIENCE PREFERRED:

  • Bachelor’s degree or Master’s degree in Nursing

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

  • Current Florida RN License
