GENERAL SUMMARY: Provide oversight and direction for Acute Care Management services, ensuring strategic and operational imperatives are successful. As part of the multi-campus leadership, communicates effectively with executives and peers. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. PRINCIPAL DUTIES AND JOB RESPONSIBILITIES: • Designs, establishes, and maintains an organizational structure and staffing to effectively accomplish the organization's goals and objectives; recruits, trains, supervises, and evaluates staff. • Develops and maintains annual budgets for the organization and performs periodic cost and productivity analyses. • Responsible for deployment of Strategic and Operational Imperatives that align to the one plan. • Monitors and analyzes outpatient reports and identifies trends. Develop and execute solutions to improve all resource management. • Facilitates and implements team process problem resolution regarding complex patient issues. • Monitors all high risk/high cost patients in regard to care delivery, referrals, etc. • Identifies opportunities for the development of new care management approaches. • Maintains positive relations acting as liaison to ensure collaboration across multiple disciplines. • Exercises independent decision-making processes to identify, plan, and execute goals. • Delegates deployment of imperatives to campus operational managers. • Accountable for maintaining consistency with standardized processes including adherence to regulatory guidelines. • Responsible for attracting & developing staff and implementing effective performance management process of staff. KNOWLEDGE AND SKILLS REQUIRED: • Excellent organizational, analytical and problem-solving skills • Effective oral and written communication skills, with the ability to articulate complex information in understandable terms to all levels of staff • Effective computer skills, particularly Microsoft Office Outlook, Word, Excel, PowerPoint. • Ability to work in a matrix-management environment to achieve organizational goals. • Understanding the community care management program. • Innovation of best practices, process development and management, change management KNOWLEDGE AND SKILLS PREFERRED: • Knowledge of Cerner; NaviHealth; FirstLook; Qlikview, and EMR • Knowledge and skill in using CNO dashboard EDUCATION AND EXPERIENCE REQUIRED: • Bachelor’s Degree in Nursing OR • Bachelor of Social Work AND Master’s Degree in a related field • 2 years of progressive leadership experience • 5 years of experience in the functional area EDUCATION AND EXPERIENCE PREFERRED: • N/A LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED: • N/A LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED: • Florida Department of Health Nursing License | ||
